Preparing for your Next Dental Appointment as a New Patient

Welcome to [clinic name]! We are happy to have you join our team of happy patients. Preparing for your first appointment with our dental care team is a bit more extensive. We love to learn about you, your oral health, and your goals for your smile. See below for the steps for you to take as a new patient of Dr. Chow.

Step 1: Schedule your appointment

Connect with one of our receptionists by calling (250) 382-5233 or by using our appointment request form by clicking HERE. We are happy to schedule your first appointment at your earliest convince. While scheduling with us over the phone or online, we will confirm some of the steps below so you are ready for your first appointment with our expert team. We will also confirm your appointment with you beforehand in case any changes have been made to your schedule.

Step 2: Download and complete forms

As our new patient, we need to learn a little more about you and your smile. To save yourself some time on your appointment date, you can download our new patient forms and complete them at your pace at home. Click below to download our new patient forms

DOWNLOAD NEW PATIENT FORMS

If you don’t have a printer or the time, it’s not a problem! Just arrive for your appointment 10 to 15 minutes before your scheduled time so you can complete the forms before our hygienists and dentists see you.

Step 3: Share your dental history

One of our new patient forms is dedicated to your dental history. With an oral exam and dental X-rays, our dentists can see some of the treatments that you have received in the past. We ask that you share past procedures, treatments, surgeries, and other events that have occurred. At your first appointment, we may confirm some of the treatments/procedures that you have had in the past. This ensures that our dentists understand your oral health and can treat you best in the future.

Step 4: Share your health/medication history

It is crucial that our dentists know which health conditions and medications (if any) that you are currently being treated for by your doctor. Please share with us the name, dosage, and how often you are taking said medication. As well as the treatment plan you are on. Some medical issues can be linked to dental issues, and some medications can cause a reaction to some treatments. We want to keep you safe, and not knowing your medical history can pose unnecessary visits and risks.

Step 5: Supply us with dental insurance information (if applicable)

We accept most types of dental insurance, including private coverage through insurance providers like plans like Sunlife, Manulife, Great West Life, Empire Life, and more. Our team will gladly provide you with an estimate (if needed) to understand the coverage that will be provided by your insurance.

Step 6: Care for your smile

Before your appointment, we ask that you care for your teeth as you normally would. This will help us understand your oral care routine. At your appointment, we can discuss ways for you to improve your at-home care routine. Before coming into our office, create a list (on paper or your phone) of any questions you might have for our hygienists or dentists. We can answer any question and provide you with the latest information on treatments or procedures that you might be interested in.
